About Your First Visit

  • Please arrive 15 minutes before your scheduled appointment to complete the patient information form if you are a new patient or have not been seen within a year.
  • Bring your photo ID and insurance card.
  • You will be asked to sign HIPAA and financial forms during your first visit.
  • Please bring a list of medications, surgeries, and health history to each visit so we may incorporate the information into your personal health record.
  • If your insurance plan requires a co-pay, it will be due at the time of the visit. We accept cash, check, Visa, or MasterCard.
  • Services not covered under insurance require full payment at the time of service.

Unable to Keep Your Appointment?

If you are unable to keep your appointment, please notify us as soon as possible. We request at least 24 hours notice so we may offer the appointment time to another patient.
